Reappraisal of giant basal cell carcinoma: clinical features and outcomes
Giant basal cell carcinoma (GBCC) is a rare subgroup of basal cell carcinomas with diameter >5cm. Current evidence about determining factors is conflicting, suggesting patient neglect on one hand, and biologically aggressive behavior on the other, with outcomes varying from clearance to death. We aimed to clarify the natural history of GBCC and its response to treatment.

Bringing the end-of-the-bed assessment to the multidisciplinary team in outpatient pre-tibial laceration management
Five in 1000 UK emergency department attendances are pre-tibial lacerations, in frail, older patients.1, 2 Hospital admission is expensive, with associated morbidity and dependence.3 Mobilisation does not impede pre-tibial laceration healing, so community convalescence is encouraged.1 For wounds deemed unlikely to heal within 4-6 weeks, due to non-viable tissue or their depth and area, surgery is facilitated by outreach nurses, reporting to a weekly, consultant-led multi-disciplinary team (MDT).1...

Re: The use of indocyanine green angiography in postmastectomy reconstruction: Do outcomes improve over time?
We read with interest the paper by Diep et al.1 The authors provide a retrospective review of Indocyanine green angiography (ICGA) use in two-stage implant-based immediate reconstruction comparing short-term complication rate in 135 patients over three different time points stratified into three equally sized groups. ICGA was used to determine areas of perfusion less than 20% to undergo debridement, prior to placement of the expander. The authors demonstrate a reduction in the incidence of mild skin...

A simple, effective, universal, and reusable osteotomy tool for jaw reconstructions with microvascular fibula-transplants
Precise, expensive individual saw guides are used with increasing frequency for the reconstruction of mandibular defects with fibular grafts. In this report, an alternative is presented – the Multiuse Cutting Jig (MUC-Jig, proprietary development). It is reusable, suitable for all patients, requires simple planning based on conventional CT imaging, and is more economical.

Response to: Keystone island flaps for reconstruction following lower leg skin cancer resection: A comparison with split-thickness skin grafts.
We were very interested to read the correspondence by Sorooshian et al in the March 2019 edition of JPRAS, comparing the use of keystone flaps and skin grafts for lower limb skin cancer reconstruction.1 The keystone perforator flap is widely reported as a robust and versatile local fasciocutaneous flap, with applications in the reconstruction of defects in the head and neck, within burns and over joints.5 Despite consensus that local flaps provide superior cosmetic and functional outcomes compared...

Adipose-derived stem/progenitor cells from lipoaspirates: A comparison between the LipiVage200-5 liposuction system and the Body-Jet liposuction system
Adipose-derived stem/progenitor cells (ADSPCs) are under investigation in many clinical applications for their regenerative potential in a variety of autoimmune, degenerative and inflammatory diseases. Adipose tissue, which is mainly harvested by manual liposuction, is the main source of these ADSPCs.

Delayed two-stage breast reconstruction – the impact of radiotherapy
Despite a trend towards immediate breast reconstruction in recent years, delayed breast reconstruction using a tissue expander is still a common procedure. Radiotherapy after mastectomy but prior to reconstruction is a risk factor, although studies examining the effect of this are limited. The aim of this retrospective cohort study is to evaluate the impact of pre-reconstructive radiotherapy (PRT) in patients undergoing breast reconstruction using an expander/implant.

Subtle Eyelid Retraction after Lower Blepharoplasty
Lower blepharoplasty is one of the commonest cosmetic surgeries performed in the United States. The delicate balance of the lower eyelid may be detrimentally altered in lower blepharoplasty, leading to lower eyelid retraction with the attendant functional and cosmetic consequences. Marginal reflex distance-2 (MRD2) is an insensitive measure for subtle lower eyelid retraction, and the MRD2 at the lateral limbus (MRD2limbus) and tarsal marginal show (TMS) may be more sensitive in identifying eyelid...

The Current Role of Sentinel Lymph Node Biopsy in the Management of Cutaneous Melanoma - a UK consensus statement
Sentinel node biopsy (SNB) has been at the forefront of the surgical staging of melanoma patients for the past 15 years. The high accuracy of this prognostic staging procedure is now recognised in all international guidelines for melanoma. However during this period there have been a number of important changes in the management of melanoma, many occurring within the past five years. The outcomes of five recent randomised Phase 3 trials have established the role of adjuvant targeted therapy and immunotherapy...

Effects of vasopressors on circulation in the porcine abdominal island flap model
During reconstructive surgical procedures, systemic vasopressors are frequently used to maintain normal blood pressure. However, questions have arisen regarding the pharmacologic effects of vasopressors on flap circulation. Many plastic surgeons have expressed concern about the possibility of impaired flap circulation caused by the vasoconstrictive effect of the drugs. However, the opposing argument exists that the increase of mean arterial pressure from vasoactive agents may improve flap perfusion.

Comment on: If it ain't broke, don't fix it: Ethics of splinting deformed newborn ears
I read the article entitled “If it ain't broke don't fix it? Ethics of splinting deformed newborn ears” by van Wijk et al.1 with great interest and would like to congratulate them for the publication. The article argues the ethical and social aspects of ear splinting of neonatal ear deformity, by listing the benefits and socio-ethical risks of the treatment. The authors conclude that splinting of neonatal ear deformity is ethically acceptable at an individual level because it prevents future surgical...

Cost-effectiveness of pre-pectoral implant-based breast reconstruction: a pilot comparative analysis
Implant-based breast reconstruction (IBR) is the most common reconstructive procedure after mastectomy1. Over the last decade, the introduction of Acellular Dermal Matrices (ADMs)/synthetic meshes has driven a change in practice from total submuscular implants to sub-pectoral implants. In the last 5 years, pre-pectoral approach to IBR with complete ADM/mesh cover is gaining popularity as an alternative to sub-pectoral IBR given its reported benefits in terms of postoperative pain, animation deformity...

Classification Pattern and Step-by-Step Procedure for Cartilage Grafts with Silicone Implants for Nasal Tip Plasty in Asians
Dorsal augmentation using silicone and tip plasty using autogenous cartilages is commonly performed in Asians. No study has investigated the classification pattern and step-by-step procedure for tip plasty using silicone implants. Therefore, this study classified cartilage grafts using silicone implants in Asians and developed a step-by-step procedure for their implementation. The study included 39 patients who had undergone augmentation rhinoplasty with a silicone implant combined with a conchal...

Pure skin Perforator Flap direct elevation along the subdermal plane using preoperative ultra-high frequency ultrasound planning: a proof of concept
Perforator flaps represents nowadays a gold standard in reconstructive surgery. However, when the aim of the reconstruction is resurfacing rather than creating volume or filling spaces, traditional perforator flaps may result in suboptimal results due to their bulkiness.

Plastic surgery trainees, GDPR and the eLogbook- are we doing it all wrong?
The Pan-Surgical eLogbook is the primary logbook for plastic surgery trainees in the UK. The Joint Committee for Surgical Training expects trainees to keep their eLogbook updated throughout training in order to demonstrate the surgical competences required for completion of training.1 At the time of writing, over 43 million operations have been logged in eLogbook by 33,000 surgeons of all specialties. With this amount of patient data stored outside NHS servers, it is important to consider our duties...
