Comment on “Sphincter-Sparing Excision and Reconstruction Using Facial Artery Perforator Flaps for Lower Lip Carcinoma” by Aksam et al

We read with interest the article by Aksam et al.1 First of all, we congratulate the authors on their fine work. We use similar perforator flaps in facial reconstruction.2 In our experience, it is possible to use the modiolus perforator flap from the angle of the mouth to reconstruct defects in the upper or lower lip.3 For lip reconstruction in men, we prefer to abstain from using flaps harvested from the marionette line.3 It is possible to avoid the beard-bearing tissue caudal to the mouth angle by instead raising the flap cranially from the nasolabial fold3 (Fig 1).
