Salmonella enterica subsp. arizonae is a bacteria commonly found in the gut of reptiles. In humans, infections caused by this organism are rare. Most cases originate from southwestern United States, where rattlesnake products are often used in traditional medicine. In Asia, only a few cases have been described. This case report documents a case involving a 64-year-old woman with pyelonephritis caused by S. arizonae in Japan. She had no history of contact with reptiles or foreign travel. The likely route of transmission is unclear. She was treated with cephalosporins for 14 days and the pyelonephritis appeared to be resolved; however recurrence occurred twice –after two weeks and then after one month. Prolonged antibiotic therapy with amoxicillin resolved the infection. This case demonstrates that pyelonephritis associated with S. arizonae can be found outside of the typical geographic region and may not be associated with typical animal hosts.

IntroducciónLa fístula faringocutánea es la más frecuente de las complicaciones poslaringectomía total. Su incidencia varía entre un 9-25% en pacientes poslaringectomía total primaria, hasta un 14-57% en laringectomía de rescate posradioterapia o posquimioterapia+radioterapia. El colgajo de músculo pectoral mayor miofascial (PMMF) se postula como una herramienta útil para disminuir la incidencia de esta complicación.Material y métodoAnálisis retrospectivo de pacientes tratados mediante laringectomía total de rescate, asociada o no a refuerzo de sutura faríngea con colgajo de PMMF.ResultadosVeinte pacientes fueron incluidos, 18 hombres (90%) y 2 mujeres (10%), en 10 de los cuales se utilizó colgajo de PMMF. La edad promedio fue de 66,65 años. Diecisiete (85%) presentaron un tumor laríngeo y 3 (15%) un tumor de hipofaringe. Ocho (80%) pacientes del grupo sin PMMF presentaron fístula en el postoperatorio, mientras que tan solo 2 (20%) pacientes del grupo de PMMF presentaron fístula durante el postoperatorio (p=0,005). El tiempo promedio para el cierre de la fístula fue significativamente menor en los casos en que se empleó el PMMF (16±11 vs. 76,8±67 días, p=0,001), así como la estancia hospitalaria (19,6±18 vs. 83,9±77 días, p0,001).ConclusiónEl uso del PMMF se asocia a una menor tasa de fístulas poslaringectomía total de rescate en pacientes tratados de forma primaria mediante protocolo de conservación de órgano por cáncer de laringe/hipofaringe, y favorece la cicatrización local disminuyendo el tiempo promedio de cierre de fístulas y la estancia media hospitalaria.IntroductionPharyngocutaneous fistula is the most frequent complication after total laryngectomy. The mean time for fistula closure was significantly shorter in the cases where PMMF flap was used (16±11 days vs. 76.8±67 days, P=.001), as was hospital stay (19. 6±18 days vs. 83.9±77 days, P=.001).ConclusionThe use of PMMF in our series is associated with a lower rate of post salvage laryngectomy fistulas in patients treated primarily by organ preservation protocol for laryngeal/hypopharyngeal cancer. In turn, it promotes local healing by decreasing the mean duration of fistula closure and the mean hospital stay.

oVEMP as an objective indicator of successful repositioning maneuver

Background and objectiveBenign paroxysmal positioning vertigo (BPPV) is the most common peripheral vestibular disorder. Canalolithiasis in the posterior semi-circular canal is the most common underlying pathology that can be treated effectively by repositioning maneuvers. Our hypothesis suggested that successful maneuvers can lead to repositioning of dislodged otoconia to the utricle.Materials and methodsAir conducted oVEMP, which is thought to originate from the contra-lateral utricular organ was measured in twenty patients with unilateral BPPV and we compared n1–p1 peak to peak amplitude of the affected ears in 3 separate intervals: on pre-treatment when typical nystagmus was confirmed, immediately after, and 1 week after repositioning maneuvers to assess change, if any, in amplitude.ResultsThis study showed significant increase of oVEMP amplitude in the affected ears after successful repositioning maneuver that was more significant after 1 week.ConclusionsoVEMP can be used as a reliable objective test for ensuring a successful maneuver rather than subjective dependence on the patient's symptoms, which may be misleading due to a remission.

Introducción y objetivosEn los últimos años se ha incrementado el uso de medidas cepstrales para la evaluación acústica de la voz. Uno de los parámetros más investigados es la prominencia del pico cepstral suavizado (CPPs). Los objetivos de este trabajo son conocer la utilidad de esta medida acústica en la evaluación objetiva de las alteraciones de la voz en español y determinar qué tipo de muestra de voz (vocales sostenidas o habla conectada) es la más sensible para evaluar la severidad de la disfonía.MétodoEn este estudio participaron 40 sujetos, 20 normofónicos y 20 con disfonía. Se grabaron 2 muestras de voz para cada sujeto (una/a/sostenida y 4 frases fonéticamente balanceadas) y se calculó la CPPs con el programa Praat. Tres expertos valoraron perceptivamente la voz de la muestra con el parámetro grado de la escala GRABS.ResultadosSe encontraron valores significativamente menores en las voces disfónicas, tanto para la/a/(t[38]=4,85, p<0,000) como para las frases (t[38]=5,75, p<0,000). En relación con el tipo de muestra de voz más indicado para evaluar la severidad de las alteraciones de la voz se encontró una fuerte correlación con la escala acústico-perceptiva de la CPPs calculada a partir del habla conectada (rs = –0,73) y moderada con la calculada a partir de la vocal sostenida (rs=–0,56).ConclusiónLos resultados de este estudio preliminar apuntan a que la CPPs es una buena medida para detectar la disfonía y para evaluar objetivamente el grado de severidad de las alteraciones en la voz.Introduction and objectivesIn recent years, the use of cepstral measures for acoustic evaluation of voice has increased. Two voice samples were recorded for each subject (one sustained vowel/a/and four phonetically balanced sentences) and the CPPs was calculated using the Praat programme. Three raters perceptually evaluated the voice sample with the Grade parameter of GRABS scale.ResultsSignificantly lower values were found in the dysphonic voices, both for/a/(t[38]= 4.85, P<.000) and for phrases (t[38] = 5,75, P<.000). An integrated approach to identify critical transcription factors in the protection against hydrogen peroxide-induced oxidative stress by Danhong injection

Oxidative stress plays a vital role in many pathological processes of the cardiovascular diseases. However, the underlying mechanism remains unclear, especially on a transcription factor (TF) level. In this study, a new method, concatenated tandem array of consensus transcription factor response elements (catTFREs), and an Illumina-based RNA-seq technology were integrated to systematically investigate the role of TFs in hydrogen peroxide (H2O2)-induced oxidative stress in cardiomyocytes; the damage was then rescued by Danhong injection (DHI), a Chinese standardized product approved for cardiovascular diseases treatment. The overall gene expression revealed cell apoptosis and DNA repair were vital for cardiomyocytes in resisting oxidative stress. By comprehensively integrating the transcription activity of TFs and their downstream target genes, an important TFs-target network were constructed and 13 TFs were identified as critical TFs in DHI-mediated protection in H2O2-induced oxidative stress. By using the integrated approach, seven TFs of these 13 TFs were also identified in melatonin-mediated protection in H2O2-induced damage. Furthermore, the transcription activity of DNA-(apurinic or apyrimidinic site) lyase (Apex1), Myocyte-specific enhancer factor 2D (Mef2d) and Pre B-cell leukemia transcription factor 3 (Pbx3) was further verified in pluripotent stem cell-derived cardiomyocytes. This research offers a new understanding of cardiomyocytes in response to H2O2-induced oxidative stress and reveals additional potential therapeutic targets. The combination of two parallel omics datasets (corresponding to the transcriptome and proteome) can reduce the noise in high-throughput data and reveal the fundamental changes of the biological process, making it suitable and reliable for investigation of critical targets in many other complicated pathological processes.

Potential role of ferric hemoglobin in MS pathogenesis: Effects of oxidative stress and extracellular methemoglobin or its degradation products on myelin components

There is a well-documented relationship between cerebral vasculature and multiple sclerosis (MS) lesions: abnormal accumulations of iron have been found in the walls of the dilated veins in cerebral MS plaques. The source of this iron is unknown, but could be related to the recognized phenomenon of capillary and venous hemorrhages leading to blood extravasation. In turn, hemorrhaging leading to hemolysis results in extracellular release of hemoglobin, a reactive molecule that could induce local oxidative stress, inflammation, and tissue damage. Our previous studies with a reduced form of hemoglobin (oxyHb) have demonstrated its ability to cause extensive lipid and protein oxidation in vitro, which would result in membrane destabilization. Here, we investigated in further detail the mechanism by which the more abundant oxidized form of extracellular hemoglobin (metHb), and dissociated hemin, cause direct oxidative damage to myelin components, specifically membrane-mimetic lipid vesicles and myelin basic protein (MBP), a highly-abundant protein in the CNS. Oxidation of lipids was assessed by the formation of conjugated diene/triene and malondialdehyde, and oxidation of MBP was demonstrated by the bityrosine formation and by the change in protein mass. Our results show that metHb causes oxidative damage to MBP and myelin lipids, partly by transferring its hemin moiety to protein and lipid, but mostly as an intact protein possibly via formation of a ferryl radical. These results elucidating the mechanism of extracellular hemoglobin-induced oxidative damage to myelin components support the need for further research into vascular pathology in MS pathogenesis, to gain insight into the role of iron deposits and/or in stimulation of different comorbidities associated with the disease.

Quantitative Mass Spectrometry Reveals Food Intake-Induced Neuropeptide Level Changes in Rat Brain: Functional Assessment of Selected Neuropeptides as Feeding Regulators [Research]
Endogenous neuropeptides are important signaling molecules that function as regulators of food intake and body weight. Previous work has shown that neuropeptide gene expression levels in a forebrain reward site, the nucleus accumbens (NAc), were changed by feeding. In order to directly monitor feeding-induced changes in neuropeptide expression levels within the NAc, we employed a combination of cryostat dissection, heat stabilization, neuropeptide extraction and label-free quantitative neuropeptidomics via a liquid chromatography-high resolution mass spectrometry platform. Using this methodology, we described the first neuropeptidome in NAc and discovered that feeding caused the expression level changes of multiple neuropeptides derived from different precursors, especially proSAAS-derived peptides such as Big LEN, PEN and little SAAS. We further investigated the regulatory functions of these neuropeptides derived from the ProSAAS family by performing an intra-NAc microinjection experiment using the identified ProSAAS neuropeptides, 'Big-LEN' and 'PEN'. Big LEN significantly increased rats' food and water intake, whereas both big LEN and PEN affected other behaviors including locomotion, drinking and grooming. In addition, we quantified the feeding-induced changes of peptides from hippocampus, hypothalamus and striatum to reveal the neuropeptide interplay among different anatomical regions. In summary, our study demonstrated neuropeptidomic changes in response to food intake in the rat NAc and other key brain regions. Importantly, the microinfusion of ProSAAS peptides into NAc revealed that they are behaviorally active in this brain site, suggesting the potential utility of these peptides as therapeutics for eating disorders.

Apoptosis induced by Moringa oleifera Lam. pod in mouse colon carcinoma model
Abstract
The aim of this study was to investigate the effect of boiled Moringa oleifera Lam. pod (bMO) in colon carcinogenesis via apoptotic pathway. Four groups of mice (eight mice/group) were induced by azoxymethane (AOM, 10 mg/kg BW i.p. at the third week) followed by dextran sodium sulfate (DSS) for 7 days (2% in drinking water at the fourth week), while one negative group (n = 8) received AIN-76A basal diet without AOM/DSS induction. Positive control group received basal diet with AOM/DSS induction, whereas treatment groups received basal diet containing 1.5%, 3.0%, and 6.0% of bMO at the 5th week till 20th week after AOM/DSS inductions. Mice were sacrificed and necropsied at the end of the 20th week. The results showed tumor incidence 87.5% and multiplicity 9.5 in positive control group. Subsequently, mice that received 1.5%–6.0% bMO exhibited tumor incidences and multiplicities at 25%–75% and 1.8–8.3, respectively. Pro-apoptosis protein BAX investigated by immunohistochemistry in mice treated with 3.0% bMO was significantly higher than positive control. Anti-apoptosis protein BCL-2 showed the lowest expression in mice fed with 6.0% bMO. The BAX/BCL-2 ratio proteins and the nuclear staining were found at the highest rate in mice received 3.0% bMO. This finding suggested that the bMO exhibited potent apoptosis induction through dissimilarity of BCL-2 family protein expression, thereby decreased BCL-2 protein expression and increased the BAX protein to promote cell death. Therefore, bMO has a potential as chemotherapeutic agent in colon cancer through induction of apoptosis.
