Increased 99mTc Pertechnetate Uptake But Unimpressive 131I Activity in the Metastatic Mediastinal Lymph Nodes in a Thyroid Cancer Patient

False negative 131I accumulation in metastatic lesions of thyroid carcinoma could be due to various etiologies. Here we present a 33-year-old woman with a history of papillary thyroid cancer who had increased 99mTc pertechnetate activity in the mediastinal metastatic lymph nodes, which was not clearly seen on whole body 131I scan due to adjacent activity from lung metastases.
