Effects of late-night short-sleep on in-home polysomnography: relation to adult age and sex

Summary

Bedtime is frequently delayed by many factors in life, and a homeostatic response to the delay may compensate partly for increased time awake and shortened sleep. Because sleep becomes shorter with age and women complain of disturbed sleep more often than men, age and sex differences in the homeostatic response to a delayed bedtime may modify the homeostatic response. The purpose of the present study was to investigate the effect of late-night short-sleep (3 h with awakening at about 07:00 hours) on in-home recorded sleep in men and women in two age groups (20–30 and 65–75 years). Results (N = 59) showed that late-night short-sleep was associated with an increase in percentage of N3 sleep and a decrease in percentage of rapid eye movement sleep, as well as decreases in several measures of sleep discontinuity and rapid eye movement density. Men showed a smaller decrease in percentage of rapid eye movement sleep than women in response to late-night short-sleep, as did older individuals of both sexes compared with younger. Older men showed a weaker percentage of N3 sleep in response to late-night short-sleep than younger men. In general, men showed a greater percentage of rapid eye movement sleep and a lower percentage of N3 sleep than women, and older individuals showed a lower percentage of N3 sleep than younger. In particular, older men showed very low levels of percentage of N3 sleep. We conclude that older males show less of a homeostatic response to late-night short-sleep. This may be an indication of impaired capacity for recovery in older men. Future studies should investigate if this pattern can be linked to gender-associated differences in morbidity and mortality.

Magnetic resonance imaging of the upper airway in patients with quadriplegia and obstructive sleep apnea

Summary

The aim of this study was to investigate upper airway anatomy in quadriplegics with obstructive sleep apnea. Fifty subjects were recruited from three hospitals in Australia: people with quadriplegia due to spinal cord injury and obstructive sleep apnea (= 11), able-bodied people with obstructive sleep apnea (= 18), and healthy, able-bodied controls (= 19). All underwent 3-Tesla magnetic resonance imaging of their upper airway. A subgroup (= 34) received a topical vasoconstrictor, phenylephrine and post-phenylephrine magnetic resonance imaging. Mixed-model analysis indicated no significant differences in total airway lumen volume between the three groups (= 0.086). Spinal cord injury–obstructive sleep apnea subjects had a significantly larger volume of soft palate (P = 0.020) and retroglossal lateral pharyngeal walls (= 0.043) than able-bodied controls. Able-bodied–obstructive sleep apnea subjects had a smaller mandible volume than spinal cord injury–obstructive sleep apnea subjects and able-bodied control subjects (= 0.036). No differences were seen in airway length between groups when controlling for height (= 0.055). There was a marginal increase in velopharyngeal volume across groups post-phenylephrine (= 0.050), and post hoc testing indicated the difference was confined to the able-bodied–obstructive sleep apnea group (< 0.001). No other upper airway structures showed significant changes with phenylephrine administration. In conclusion, people with obstructive sleep apnea and quadriplegia do not have a structurally smaller airway than able-bodied subjects. They did, however, have greater volumes of soft palate and lateral pharyngeal walls, possibly due to greater neck fat deposition. The acute response to upper airway topical vasoconstriction was not enhanced in those with obstructive sleep apnea and quadriplegia. Changes in upper airway anatomy likely contribute to the high incidence in obstructive sleep apnea in quadriplegic subjects.

Cryptococcus neoformans Epidemiological Cutoff Values

Abstract

Purpose of Review

The purpose of this review is to provide a current view of the importance of the determination and use of epidemiological cutoff values (ECVs) for Cryptococcus neoformans, since there are no clinical breakpoints (CBPs).

Recent Findings

ECVs have been proposed for some antifungal agents and C. neoformans, using standardized methodologies by the Clinical and Laboratory Standards Institute (CLSI) and by the European Committee for Antimicrobial Susceptibility Testing (EUCAST), based on the distribution of minimum inhibitory concentrations (MICs). There is no sufficient evidence for the determination of ECVs for C. neoformans using commercial methods; however, as these methods are routinely used in the microbiology laboratory, it is recommended for the establishment of local ECVs using these methods and following the criteria for their determination.

Summary

Due to the geographic and genetic variations inherent to C. neoformans, it is important to calculate ECVs, since they are useful in clinical practice to guide therapy in the absence of CBPs.

The tympanic region of Otaria byronia (Otariidae, Carnivora) – morphology, ontogeny, age classes and dimorphism

Abstract

Here we describe and explore for the first time the ontogeny and sexual dimorphism of the auditory region of Otaria byronia. We studied the tympanic region of skulls of 237 specimens of different ages and sexes. Geometric morphometric methods were used to analyze the tympanic bulla. In addition, 3D reconstructions of the tympanic bulla were performed using computed tomography analysis scans and a serial wearing technique. We provide a description of the external and internal morphology of the tympanic bulla in both sexes and across different stages (bioclasses). The average shape of the bulla in O. byronia has a subtriangular contour, with variations between sexes and ages. Each stage (bioclasses I, II, and III) is characterized by the respective mean shape of the tympanic bulla and designated as a morphoclass (1, 2, and 3). In all cases, the ectotympanic shows greater surface area than the endotympanic, as in other otariids, in contrast to Phocidae. During ontogeny, the relative size of the ectotympanic increases, growing in all directions and covering the endotympanic. This pattern is seen to the greatest extent in adult males, in which the ectotympanic forms an extremely well-developed apophysis jugulare. No differences in internal morphology of the tympanic cavity were recorded between ages and sexes. The bulla does not increase in thickness in successive age classes; in fact, the walls are extremely thin in the adult stages, despite the extensive development of its processes. This pattern is opposite that observed in Phocidae. In morphoclass 3, adult males older than 7 years undergo hypermorphic change that results in a peramorphic condition when compared to adult females. These changes probably follow the same pattern shown by the rest of the skull and contribute to the marked sexual dimorphism of the species.

Immediately restored full arch-fixed prosthesis on implants placed in both healed and fresh extraction sockets after computer-planned flapless guided surgery. A 3-year follow-up study

Abstract

Background

The treatment of patients by the use of immediate implant placement in post-extractive site is a challenging procedure.

Purpose

A 3-year clinical and radiological study of post-extractive implants placed using flapless guided surgery and immediately functioning.

Materials and Methods

Thirty-two patients (23 females and 9 males), aged between 44 and 73 years (a mean age of 59.5) were treated with immediate full arch restorations and flapless implant surgery in fresh extraction and healed sites. A double-guide technique stent in conjunction with the NobelGuide system (Nobel Biocare AB, Göteborg, Sweden) was used.

Results

A total of 285 implants over 32 patients were assessed. The patients were clinically and radiologically followed for 3 years. One hundred and ninety-five implants were placed in the maxilla and 90 in the mandible. Eight patients received implants in both arches. One hundred and ninety-seven implants were placed in extraction sites (137 maxilla, 60 mandible) and 88 in healed sites (58 maxilla and 30 mandible). The overall cumulative implant survival rate (CISR) was 97.54%. Two implants failed in maxillary healed sites (CISR 96.55%), three in maxillary extraction sites (CISR 97.81%), and two in mandibular extraction sites (CISR 96.66%). No implant failed in healed mandibular sites (CSR 100%). All fixed prostheses maintained stability and good functionality during the follow-up, accounting for a cumulative prosthesis survival rate (CPSR) of 100%. The overall marginal bone level (MBL) was −0.52 mm (SD −0.18) after 6 months, −0.88 mm (SD −0.20) after 12 months, −1.05 mm (SD −0.21) after 24 months, and −1.32 mm (SD −0.41) after 36 months.

Conclusions

Computer-guided surgery using double-template technique (DTT) shows a predictable outcome in the medium term, decreasing treatment timing and patient discomfort.

Abstract

Mediation analysis helps researchers assess whether part or all of an exposure's effect on an outcome is due to an intermediate variable. The indirect effect can help in designing interventions on the mediator as opposed to the exposure and better understanding the outcome's mechanisms. Mediation analysis has seen increased use in genome-wide epidemiological studies to test for an exposure of interest being mediated through a genomic measure such as gene expression or DNA methylation (DNAm). Testing for the indirect effect is challenged by the fact that the null hypothesis is composite. We examined the performance of commonly used mediation testing methods for the indirect effect in genome-wide mediation studies. When there is no association between the exposure and the mediator and no association between the mediator and the outcome, we show that these common tests are overly conservative. This is a case that will arise frequently in genome-wide mediation studies. Caution is hence needed when applying the commonly used mediation tests in genome-wide mediation studies. We evaluated the performance of these methods using simulation studies, and performed an epigenome-wide mediation association study in the Normative Aging Study, analyzing DNAm as a mediator of the effect of pack-years on FEV1.

Systematic analysis reveals a lncRNA-mRNA co-expression network associated with platinum resistance in high-grade serous ovarian cancer

Summary

Resistance to platinum-based chemotherapy is the major barrier to treating high-grade serous ovarian cancer (HGS-OvCa). To improve HGS-OvCa patient prognosis, it is critical to identify the underlying mechanisms that promote platinum resistance. The goal of the present study was to identify a lncRNA-mRNA co-expression network and key lncRNAs that predict resistance to platinum-based chemotherapy in ovarian cancer patients. By systematically analyzing the expression profiles of lncRNAs and mRNAs in HGS-OvCa samples from the Cancer Genome Atlas (TCGA), we revealed that lncRNAs play important roles in platinum resistance in HGS-OvCa patients and delineate a lncRNA-mRNA co-expression network in HGS-OvCa patients who exhibit platinum resistance. Within the platinum resistance-specific lncRNA-mRNA network, 35 lncRNAs and 270 mRNAs showed 124 significant lncRNA-mRNA co-expression relationships. Pathway analysis revealed that lncRNAs in the platinum resistance network may participate in platinum resistance by regulating metabolic pathways. Moreover, HGS-OvCa patients with low lncRNA RP5-1120P11.1 expression showed a poorer prognosis than those with high lncRNA RP5-1120P11.1 expression in TCGA dataset (P = 2.74 × 10−5, log rank test), which was also validated in the GSE63885 dataset (P = 0.0242, log rank test). Network and function analysis revealed that lncRNA RP5-1120P11.1 regulates many cancer-related signaling pathways, such as the PI3K-AKT signaling pathway (P = 1.02 × 10−5, hypergeometric test) and the Jak-STAT signaling pathway (P = 1.71 × 10−4, hypergeometric test). Particularly, lncRNA RP5-1120P11.1 expression is significantly positively correlated with ABCC10 gene expression (P = 3.89 × 10−3, Pearson correlation test). Both lncRNA RP5-1120P11.1 and ABCC10 were down-regulated in platinum-resistant HGS-OvCa patients, and RP5-1120P11.1 is located near ABCC10 on chromosome 6. Gene ABCC10 has been implicated in resistance to docetaxel treatment. The present study paves the way for investigating lncRNA functions in platinum drug resistance and identifying lncRNAs with prognostic and therapeutic potential in HGS-OvCa.

Conformational memory in the association of the transmembrane protein phospholamban with the sarcoplasmic reticulum calcium pump SERCA [Membrane Biology]

The sarcoplasmic reticulum Ca2+-ATPase SERCA promotes muscle relaxation by pumping calcium ions from the cytoplasm into the sarcoplasmic reticulum. SERCA activity is regulated by a variety of small transmembrane peptides, most notably by phospholamban in cardiac muscle and sarcolipin in skeletal muscle. However, how phospholamban and sarcolipin regulate SERCA is not fully understood. In the present study, we evaluated the effects of phospholamban and sarcolipin on calcium translocation and ATP hydrolysis by SERCA under conditions that mimic environments in sarcoplasmic reticulum membranes. For pre-steady state current measurements, proteoliposomes containing SERCA and phospholamban or sarcolipin were adsorbed to a solid-supported membranes and activated by substrate concentration jumps. We observed that phospholamban altered ATP-dependent calcium translocation by SERCA within the first transport cycle, while sarcolipin did not. Using pre-steady-state charge (calcium) translocation and steady-state ATPase activity under substrate conditions (various calcium and/or ATP concentrations) promoting particular conformational states of SERCA, we found that the effect of phospholamban on SERCA depends on substrate preincubation conditions. Our results also indicated that phospholamban can establish an inhibitory interaction with multiple SERCA conformational states, with distinct effects on SERCA's kinetic properties. Moreover, we noted multiple modes of interaction between SERCA and phospholamban, and observed that once a particular mode of association is engaged it persists throughout the SERCA transport cycle and multiple turnover events. These observations are consistent with conformational memory in the interaction between SERCA and phospholamban, thus providing insights into the physiological role of phospholamban and its regulatory effect on SERCA transport activity.

Obituary for Professor Hugo L. Obwegeser

One of the most comprehensive, influential, and illustrious careers in the history of oral and maxillofacial surgery came to conclusion on September 2, 2017, with the death of Hugo L. Obwegeser, in Zurich. Professor Obwegeser had been in deteriorating health for most of the year, and succumbed to respiratory complications some six weeks short of his 97th birthday.

Screening of Natural Bioactive Metabolites and Investigation of Antioxidant, Antimicrobial, Antihyperglycemic, Neuropharmacological, and Cytotoxicity Potentials of Litsea polyantha Juss. Ethanolic Root Extract

This study was designed to identify some bioactive phytochemicals from ethanolic extract of roots of Litsea polyantha and to evaluate some of its pharmacological activities. Phytochemical tests indicated the presence of reducing sugar, combined reducing sugar, tannins, flavonoids, alkaloids, terpenoids, and phenol. In the antioxidant assay using 2-diphenyl-1-picryl-hydrazyl (DPPH) free radical scavenging method, the IC50 value was found to be 82.31 μg/mL. Total content of phenolic compounds, flavonoid, and tannin was found to be 152.69 mg GAE/gm, 85.60 mg QE/gm, and 77.22 mg GAE/gm of dry extract, respectively. In disc diffusion antibacterial assay, the extract exhibited highest zone of inhibition up to 12.25 mm against Escherichia coli at the concentration of 500 μg/disc. For brine shrimp lethality bioassay, the extract exhibited LC50 56.082 μg/mL. In in vivo antihyperglycemic activity test by oral glucose tolerance test using Swiss Albino mice at the oral dose of 250 and 500 mg/kg, the extract showed statistically significant antihyperglycemic effect. Finally, in vivo, the extract exhibited the dose dependent CNS depressant effects by reducing the locomotors of Swiss Albino mice which was confirmed through three different neuropharmacological activity tests such as open field, hole cross, and hole board test.

miRNAs: Important Targets for Oral Cancer Pain Research

Pain is a symptom shared by an incredible number of diseases. It is also one of the primary conditions that prompt individuals to seek medical treatment. Head and neck squamous cell carcinoma (HNSCC) corresponds to a heterogeneous disease that may arise from many distinct structures of a large, highly complex, and intricate region. HNSCC affects a great number of patients worldwide and is directly associated with chronic pain, which is especially prominent during the advanced stages of oral squamous cell carcinoma (OSCC), an anatomical and clinical subtype that corresponds to the great majority oral cancers. Although the cellular and molecular bases of oral cancer pain have not been fully established yet, the results of recent studies suggest that different epigenetic mechanisms may contribute to this process. For instance, there is strong scientific evidence that microRNAs (miRNAs), small RNA molecules that do not encode proteins, might act by regulating the mechanisms underlying cancer-related pain. Among the miRNAs that could possibly interfere in pain-signaling pathways, miR-125b, miR-181, and miR-339 emerge as some of the most promising candidates. In fact, such molecules apparently contribute to inflammatory pain. Moreover, these molecules possibly influence the activity of endogenous pain control systems (e.g., opioidergic and serotonergic systems), which could ultimately result in peripheral and central sensitization, central nervous system (CNS) phenomena innately associated with chronic pain. This review paper focuses on the current scientific knowledge regarding the involvement of miRNAs in cancer pain, with special attention dedicated to OSCC-related pain.

Congenital Absence of Left Circumflex Artery: A Case Report and Review of the Literature

Congenital absence of the left circumflex artery is a rare coronary anomaly with few reported cases in the literature. These patients are usually diagnosed incidentally when they undergo coronary angiography or coronary CT to rule out underlying coronary artery disease. In this article, we report a case of a 46-year-old man who was incidentally found to have a congenitally absent left circumflex artery with a superdominant right coronary artery after a workup was initiated for frequent premature ventricular contractions and regional wall motion on echocardiogram. A review of the clinical presentation, symptoms, and diagnostic modalities used to diagnose this entity is presented.

Should Skin Biopsies Be Performed in Patients Suspected of Having Parkinson’s Disease?

In patients with Parkinson's disease (PD), the molecularly misfolded form of α-synuclein was recently identified in cutaneous autonomic nerve fibers which displayed increased accumulation even in early disease stages. However, the underlying mechanisms of synucleinopathic nerve damage and its implication for brain pathology in later life remain to be elucidated. To date, specific diagnostic tools to evaluate small fiber pathology and to discriminate neurodegenerative proteinopathies are rare. Recently, research has indicated that deposition of α-synuclein in cutaneous nerve fibers quantified via immunohistochemistry in superficial skin biopsies might be a valid marker of PD which could facilitate early diagnosis and monitoring of disease progression. However, lack of standardization of techniques to quantify neural α-synuclein deposition limits their utility in clinical practice. Additional challenges include the identification of potential distinct morphological patterns of intraneural α-synuclein deposition among synucleinopathies to facilitate diagnostic discrimination and determining the degree to which structural damage relates to dysfunction of nerve fibers targeted by α-synuclein. Answering these questions might improve our understanding of the pathophysiological role of small fiber neuropathy in Parkinson's disease, help identify new treatment targets, and facilitate assessment of response to neuroprotective treatment.

Evaluating the Interference of Bicycle Traffic on Vehicle Operation on Urban Streets with Bike Lanes

Many urban streets are designed with on-street bike lanes to provide right-of-way for bicycle traffic. However, when bicycle flow is large, extensive passing maneuvers could occupy vehicle lanes and thus cause interferences to vehicle traffic. The primary objective of this study is to evaluate how bicycle traffic affects vehicle operation on urban streets with bike lanes. Data were collected on six street segments in Nanjing, China. The cumulative curves were constructed to extract traffic flow information including individual bicycle and vehicle speeds and aggregated traffic parameters such as flow and density. The results showed that as bicycle density on bike lanes continuously increases faster bicycles may run into vehicle lanes causing considerable reductions in vehicle speeds. A generalized linear model was estimated to predict the vehicle delay. Results showed that vehicle delay increases as bicycle flow and vehicle flow increase. Number of vehicle lanes and width of bike lane also have significant impact on vehicle delay. Findings of the study are helpful to regions around the world in bike infrastructure design in order to improve operations of both bicycles and vehicles.

The Roles of Thrombospondins in Hemorrhagic Stroke

Hemorrhagic stroke is a devastating cerebrovascular disease with significant morbidity and mortality worldwide. Thrombospondins (TSPs), as matricellular proteins, belong to the TSP family which is comprised of five members. All TSPs modulate a variety of cellular functions by binding to various receptors. Recently, TSPs gained attention in the area of hemorrhagic stroke, especially TSP-1. TSP-1 participates in angiogenesis, the inflammatory response, apoptosis, and fibrosis after hemorrhagic stroke through binding to various molecules including but not limited to CD36, CD47, and TGF-β. In this review, we will discuss the roles of TSPs in hemorrhagic stroke and focus primarily on TSP-1.

Chinese Herbal Medicine as an Adjunctive Therapy Ameliorated the Incidence of Chronic Hepatitis in Patients with Breast Cancer: A Nationwide Population-Based Cohort Study

We conducted a National Health Insurance Research Database-based Taiwanese nationwide population-based cohort study to evaluate whether Chinese herbal medicine (CHM) treatment decreased the incidence of chronic hepatitis in breast cancer patients receiving chemotherapy and/or radiotherapy. A total of 81171 patients were diagnosed with breast cancer within the defined study period. After randomly equal matching, data from 13856 patients were analyzed. Hazard ratios of incidence rate of chronic hepatitis were used to determine the influence and therapeutic potential of CHM in patients with breast cancer. The patients with breast cancer receiving CHM treatment exhibited a significantly decreased incidence rate of chronic hepatitis even across the stratification of age, CCI score, and treatments. The cumulative incidence of chronic hepatitis for a period of seven years after initial breast cancer diagnosis was also reduced in the patients receiving CHM treatment. The ten most commonly used single herbs and formulas were effective in protecting liver function in patients with breast cancer, where Hedyotis diffusa and Jia-Wei-Xiao-Yao-San were the most commonly used herbal agents. In conclusion, our study provided information that western medicine therapy combined with CHM as an adjuvant modality may have a significant impact on liver protection in patients with breast cancer.

Chemical Composition and Quality Characteristics of Wheat Bread Supplemented with Leafy Vegetable Powders

The study investigated the effect of supplementation of the leaf powders of Telfairia occidentalis, Amaranthus viridis, and Solanum macrocarpon on the chemical composition and the quality characteristics of wheat bread. The bread samples were supplemented with each of the vegetable leaf powders at 1%, 2%, and 3% during preparation. The bread samples were assayed for proximate composition, mineral composition, physical, sensory, and antioxidant properties using standard methods. The addition of vegetable powders significantly increased the protein (9.50 to 13.93%), fibre (1.81 to 4.00%), ash (1.05 to 2.38%), and fat (1.27 to 2.00%). Supplementation with vegetable powder however significantly decreased () the carbohydrate and moisture contents. Significant () increases were recorded for all evaluated minerals as the level of vegetable powder increased. Supplementation with vegetable powder caused significant decrease in total phenolic content, percentage DPPH inhibition, metal chelating ability, ferric reducing antioxidant power, and total antioxidant capacity. Sensory results showed that there was significant decrease in sensory qualities with increasing supplementation. This therefore suggests that bread supplemented with vegetable powder could have more market penetration if awareness is highly created.

Safety Assessment and Comparison of Sodium Selenite and Bioselenium Obtained from Yeast in Mice

Detailed safety assessment of sodium selenite and bioselenium (bio-Se) was conducted and the results were compared and discussed for the purpose of assessing safety of bio-Se for use in food applications. In this work, acute toxicity studies, micronucleus test, and sperm aberration study in mice, 30-day feeding test of mice, were conducted to evaluate the toxicity of bio-Se obtained from yeast with different fermentation time (transformative time: one month, three months, and six months), and the results were compared with that of inorganic Se (sodium selenite). LD50 of sodium selenite was calculated to be 21.17 mg/kg. LD50 of bio-Se obtained from yeast with different fermentation time was calculated to be 740.2 mg/kg, 915.3 mg/kg, and 1179.0 mg/kg, respectively. In the genotoxicity test, bio-Se did not show cytotoxicity and genotoxicity of mice while sodium selenite at all dose groups was significantly different from the negative group. In the 30-day subchronic oral toxicity study, sodium selenite may slow down the growth of the mice and lead to organic damage to some extent. Bio-Se had facilitated effect towards the body weight of the mice and had no significant effect on the shape and function of the important organs of the mice.

Source Parameter Estimation Method for Assessment of Structural Resiliencies

Assessing structural integrity and sustainability during natural hazards, for instance, strong earthquakes, is an effective way to reduce or even avoid large losses of life and property damage. With the established vulnerability relationships between source parameter and building damage, the seismic resilience of structures can be obtained after the source parameter is estimated in the early stage of an earthquake. For this purpose, we propose a method that employs the wave displacement parameter to estimate earthquake magnitude in real time to quick assess the structural resiliencies. By selecting period and amplitude parameter as comparisons, the magnitude estimation formulas are derived, respectively, where the proposed P wave displacement parameter method is of the highest precision. Through the evolutionary estimation of the P wave displacement parameter as a function of the time window used, we show that the existing regression relationships can be extended to the large earthquake. Therefore, this paper provides a quick earthquake magnitude estimation method for the establishment of a more reasonable and accurate resilience assessment system for structures.

Diagnosis of Localized Faults in Multistage Gearboxes: A Vibrational Approach by Means of Automatic EMD-Based Algorithm

The gear fault diagnosis on multistage gearboxes by vibration analysis is a challenging task due to the complexity of the vibration signal. The localization of the gear fault occurring in a wheel located in the intermediate shaft can be particularly complex due to the superposition of the vibration signature of the synchronous wheels. Indeed, the gear fault detection is commonly restricted to the identification of the stage containing the faulty gear rather than the faulty gear itself. In this context, the paper advances a methodology which combines the Empirical Mode Decomposition and the Time Synchronous Average in order to separate the vibration signals of the synchronous gears mounted on the same shaft. The physical meaningful modes are selected by means of a criterion based on Pearson's coefficients and the fault detection is performed by dedicated condition indicators. The proposed method is validated taking into account simulated vibrations signals and real ones.

A New Digital Watermarking Method for Data Integrity Protection in the Perception Layer of IoT

Since its introduction, IoT (Internet of Things) has enjoyed vigorous support from governments and research institutions around the world, and remarkable achievements have been obtained. The perception layer of IoT plays an important role as a link between the IoT and the real world; the security has become a bottleneck restricting the further development of IoT. The perception layer is a self-organizing network system consisting of various resource-constrained sensor nodes through wireless communication. Accordingly, the costly encryption mechanism cannot be applied to the perception layer. In this paper, a novel lightweight data integrity protection scheme based on fragile watermark is proposed to solve the contradiction between the security and restricted resource of perception layer. To improve the security, we design a position random watermark (PRW) strategy to calculate the embedding position by temporal dynamics of sensing data. The digital watermark is generated by one-way hash function SHA-1 before embedding to the dynamic computed position. In this way, the security vulnerabilities introduced by fixed embedding position can not only be solved effectively, but also achieve zero disturbance to the data. The security analysis and simulation results show that the proposed scheme can effectively ensure the integrity of the data at low cost.

What is a Radical Mastectomy?

Traditionally, a Radical Mastectomy called for the removal of the breast, surrounding tissue and even the chest muscle below.

A Modified Radical Mastectomy is a procedure in which the breast and surrounding tissue are removed, while leaving the chest muscle intact. In most cases, mastectomy is required in order to remove cancerous tissue from the body. The extent of tissue removed is determined by the amount of cancer present in your body.

A Modified Radical Mastectomy is one the most extensive forms of breast cancer surgery in that it calls for the complete removal not only of the breast, but of the lymph nodes as well.

Lymph nodes are small junctions that join the vessels that make up the lymphatic system. The lymphatic system circulates a bodily fluid called lymph in the same way that the circulatory system carries blood.

Your doctor has recommended that you undergo a modified radical mastectomy because the cancer in your breast may have begun to move into the lymph nodes under your arm as well as into your chest muscle.

This procedure will permanently change the outward shape and appearance of your chest.

So make sure that you ask your doctor to carefully explain the reasons behind this recommendation.

Your Procedure:

On the day of your operation, you will be asked to put on a surgical gown.

You may receive a sedative by mouth and an intravenous line may be put in.

You will then be transferred to the operating table.

In the operating room, a nurse will begin preparation by clipping or shaving your underarm.

The anesthesiologist will begin to administer anesthesia - most probably general anesthesia by injection and inhalation mask.

The surgeon will then apply an antiseptic solution to the skin and place a sterile drape around the operative site.


Two incisions will be made beginning at the middle of the chest, one along the top and one along the bottom of the breast - coming together just under the arm.

The skin is then lifted up and away, revealing the tissue underneath.

Beginning at the clavicle - or collar bone - the surgeon then begins to carefully cut the breast tissue away from the muscles that lie just beneath.

When the breast has been completely freed, it is lifted away, exposing the top layer of muscle, called the pectoralis major.

The surgeon will pull this muscle temporarily aside exposing the next layer of muscle - the pectoralis minor.

The surgeon will move this muscle aside, creating a clear view of the surrounding fatty tissue.

Within this fat deposit lie lymph nodes lymph vessels, blood vessels and nerves.

Using great care not to damage the large thoracic nerve, your doctor will remove the lymph nodes and surrounding fat.

Blood vessels will be tied off and your doctor will thoroughly examine the surrounding tissues for any other signs of disease.

When the surgical team is satisfied that they have done all that they can to remove the cancer, they will release the muscles and other tissue.

One or more drainage tubes will be temporarily inserted at the site while the healing process begins.

They will then close the incision. Finally, a sterile bandage is applied.

Inferior Petrosal Sinus Thrombosis in a Child due to Malposition ofCentral Venous Catheter: A Case Report.

Objective: To report a case of inferior petrosal sinus(IPS) thrombosis associated with malposition of central venous catheterization(CVC). Clinical Presentation and Intervention: A 5-month-old boy was admitted to Şifa University Hospital because of pneumonia. When exophtalmus occurred in the right eye, he was referred to Ege University Hospital. Cranial Magnetic Resonance Imaging (MRI) and Magnetic Resonance (MR) venography confirmed that the catheter in right IPS caused the thrombosis. The catheter was extracted and anticoagulan treatment was started. Conclusion: In this case report, malpositioning of the CVC was the cause of the thrombosis. To minimize such complications, catheterization should be done with the supervision of an expert and post-procedure radiography should also be performed.

Patients with relapsed or refractory cutaneous T-cell lymphoma (CTCL) display a dismal prognosis and their therapy represents an unmet medical need, as the best treatment strategy is yet to be determined. Exciting data on novel targeted agents are now emerging from recently concluded and ongoing clinical trials in patients with relapsed and refractory CTCL. Three FDA approved compounds are used as single agents including the oral retinoid bexarotene and histone deacetylase inhibitors romidepsin and vorinostat. Brentuximab vedotin, an anti-CD30 drug-conjugated monoclonal antibody, has received from European Commission the orphan designation but has not been approved by EMA yet. Several other molecules have demonstrated their activity in the same context and combination strategies are being explored. Participation in a well designed clinical trial is encouraged, as the introduction of novel agents will continue to expand the therapeutics options available in the management of CTCL.
