Congratulations to Starke et al on their experience with the dual microcatheter technique for wide-necked intracranial aneurysms and careful comparison with the stent-assisted approach.1 I appreciate their acknowledgement of my first conception of the technique.2 I would clarify that in our original report the technique was not used for 'rescue' as the authors state. Rather, we observed coil instability in the sac and/or coil herniation through the neck during deployment from a single microcatheter. These remain the main indications today. Its use for endovascular rescue3 following coil perforation arose later and is another good indication.
