Acquired composite mandibular defects may require a vascularized free tissue transfer to restore premorbid form and function. The fibula free flap allows for the reconstruction of hard and soft tissue defects while also providing a bone stock to utilize an endosseous implant retained or supported prostheses for functional rehabilitation of the masticatory system (1,2). Placement of the fibula along the mandibular inferior border to preserve facial contour can lead to a height discrepancy between the native alveolus and the neomandible.
